Head and neck surgery: morphological and functional characterization (CHIMERE)

Research unit - UR 7516

Description

CHIMERE is a research team born out of the surgeons’ determination to solve the hurdles and problems they were facing daily.

Its goal is to leverage the knowledge and technologies offered by fundamental sciences, academic research (anatomy, imaging, biochemistry, histology) and clinical research in order to develop innovative therapies in fields as diverse as oncology, psychology, engineering and especially surgery (robot-assisted surgery, simulation and precision surgery).

CHIMERE is a multidisciplinary research team, thematically focused on the head and neck (face, neck, brain, upper digestive tract), with an advanced imaging platform as its cornerstone. Its strong societal impact is demonstrated by the growing number of joint research projects in fields such as epidemiology, law and social sciences.

CHIMERE is an interdisciplinary team focused on the problems of innovative, precise and regenerative surgery of the cerebral extremity. The team mobilizes and invests the technological means of exploration and analysis (biology, imaging, robotics) in health and in Human and Social Sciences (psychology, history, law, art).

It is based on the gathering of complementary expertise (55 researchers) that allows a transversal approach for simultaneous and systemic consideration of clinical, biological, imaging, cognitive and behavioural data in a research-behavioural continuumindustrial development care for integrated surgery.

The interweaving of the work gives meaning to the vocation of this team devoted to the theme of regeneration and repair of tissues and organs destroyed or injured mainly by tumors, malformations or traumas of the cephalic end. The originality of his speech is based on a holistic approach (anatomy in motion, physiology, functional imaging, histology, epidemiology, psychology, cognitive sciences, etc.) surgical repair allowing morphological and functional characterization of healthy, pathological and reconstructed tissues and personalization of patient management (concept of surgical treatment "overmeasure", targeted therapy, etc.).

The activity of the Unit is divided into two axes:

1. Customization of the diagnosis and therapeutic approach of head and neck surgery:

• Customization of complex facial reconstructions and “custom” design of facial transplants:
• Clinical and surgical management of patients suffering from facial disfigurement in all its forms (tumours, traumatic sequelae, malformations, implantology, etc.).
• Optimize the surgical process with advanced and innovative techniques (robotics, imaging, hybrid reconstruction method, etc.).
• Obtaining and Characterizing Clinical Vascular Bone Substitute
• Design, characterize and validate bioartificial bone substitutes to be used as fillers for a number of facial pathologies or lesions.
• Obtain clinically-used cell matrices as a bone substitute with properties that allow cell retention and viability at the implantation site, vascular network formation, and osteogenic differentiation.
• Optimization of surgery or medical treatment of head and neck tumours: (Identify predictive biomarkers, through multiomic data analysis, of head and neck tumours to predict the success or failure of different treatments and the risk of recurrence after surgery.)

2. Morphological and functional characterization of head end pathologies:

• Physiology and pathophysiology of blood and cerebrospinal fluid discharge,
• Morphological and metabolic tumor imaging,
• Functional evaluation of clinical, physiological, imaging,
• Face-to-face: from disfigurement to refiguration, the role of the face as mediator of social interactions.

Example(s) of projects

1. FACEMOCAP/ SIMOVI: Development of an imaging
platform designed to understand, treat and manage head and neck pathologies by combining data from movement capture, eyetracking and electromyography systems.
Partners: S. DAKPE- F. SARHAN, CHU Amiens-Picardie - UTC /
Région Hauts-de-France, Gueules Cassées.

2. HANUMAN: Digital modelling of blood flow and cerebrospinal fluid hydrodynamics in order to understand craniospinal mechanics, the cause of its related
pathologies and improve diagnosis.
Partners: O. BALEDENT, Institut de mécanique des fluides (CNRS), Centre de Recherche Cerveau et Cognition (INSERM) - Université Toulouse-Paul Sabatier III, Laboratoire de Mathématiques de Reims (LMR - CNRS) - Université de Reims Champagne-Ardenne.

3. SPECTUM (Projet ANR): SpecTum is a pilot real-time
mass spectroscopy prospective analysis of mouth tumour samples in order to enable training on the utilization of SpiderMass™ instrument.
Partners: C. ATTENCOURT - S. DAKPE - A GALMICHE, CHU
Amiens-Picardie, laboratoire PRISM (Unité 1192 INSERM, Université de Lille), Canceropôle nord-ouest.

4. INPEC: Study of perineural invasion in head and neck
cancers, combining in-silico (sequencing), in-vitro (cell lines) and ex-vivo (explants) to study the relationship between head and neck epidermoid carcinomas and their
innervation.
Partners: A. GALMICHE, Ligue contre le Cancer.

5. A_TISTE: Based on artwork created with an eye-tracking
system, this project aims at comparing an artist’s eye
movement with the eye movement of an autistic patient or a patient with facial trauma, in order to develop therapeutic approaches for autistic patients or patients suffering from head and neck pathologies.
Partners : L. VANDROMME - B. DEVAUCHELLE (UPJV, CHU
Amiens-Picardie), M. PAYSANT, Maison de la Culture Amiens.
